How to make your appointment.

We make it easy to schedule appointments that work for you. To book, call or text 1-209-850-3500, or visit any Lumira Healthcare location. When you come for your appointment, bring a valid photo ID and your insurance, Medi-Cal, or Medicare card. A limited number of same-day visits are available each day, and walk-ins are welcome when space allows.

Lumira also offers free rides to and from appointments, the Lumira Healthcare Pharmacy, and referral appointments for eligible patients. If you need a ride, we’ve got you.

Insurance and Payment Assistance

We accept Medi-Cal, Medicare, Covered CA, and most private insurance plans at all of our clinic locations. For those without insurance, we offer a sliding-fee discount program based on family size and income, and free application assistance to help you qualify. We will never refuse care because of a patient’s inability to pay. Our goal is to make high-quality healthcare accessible, affordable, and free of barriers for every member of our community.

Please bring a photo ID, insurance or Medi-Cal card, immunization records, and any current medications.

Lumira Healthcare accepts plans from Medi-Cal, Medicare, Covered CA, and most insurance programs, as well as self-pay. Our self-pay sliding fee discount program is also available based on family size and income. We also offer free application assistance if you need help paying for your visit. Just bring a government-issued photo ID and pay stubs or your most recent tax return.

Yes, behavioral health services are offered at all locations. Patients must be an established medical patient in order to schedule an appointment with a counselor.

Yes, for new lab orders, patients must schedule an appointment with their provider.

Labs are sent electronically, however if you would like to obtain a copy, our receptionist can provide it to you at any of our locations.

Laboratory services offered inside our locations are currently reserved for Lumira Healthcare patients only.

No, at the moment we do not offer lab appointments. It’s first come, first serve walk-in.

Providers are available after hours for urgent questions, concerns, or emergent needs. Please call our main number 1-209-850-3500 to speak to a representative and you will receive a return call from a medical provider within one hour.

For new patients or patients that have not been seen in the last three years, a parent or legal guardian must be present. Legal guardianship forms must be provided at the time of service.

For established patients, an authorized adult 18 years or older may bring the child and must have a detailed written consent from the parent/guardian authorizing them to bring the child and a valid picture ID.

Appointments must be cancelled at least 24 hours prior to your appointment. To cancel or reschedule, simply call or text us at 1-209-850-3500.
